The Ask
Healthy Jeffco Alliance was a county-wide, cross-sector collaborative launched in 2020 to cultivate a connected, thriving community where health and opportunity are possible for all across Jefferson County, Colorado.
Aging Dynamics partnered with Healthy Jeffco Alliance from mid-2020 through early 2022 to support early development, including workgroup formation, leadership coaching, and the production of an actionable report addressing barriers and solutions to funding collaborative-led initiatives.

Producing an actionable report on the barriers and solutions to funding collaborative-led initiatives. It provides steps that community-based organizations and funders can take on their own and together to increase creativity, equity, and community power in developing and funding solutions.
2025–2026
The Ask
In 2025, the Alliance re-engaged Aging Dynamics during a pivotal moment. Due to significant funding shifts, the collaborative began exploring multiple paths for its future including restructuring, integration within Jefferson County Public Health, (the county public health department), or formal closure. Alliance leadership sought support to navigate this transition thoughtfully while honoring the shared learning, relationships, and systems change efforts that had emerged over five years.
Our Approach
Drawing on our history with the Alliance and deep understanding of its structure and partnerships, Aging Dynamics supported leadership in navigating this transition through structured facilitation, analysis, and strategic planning. Our work included guiding discussions with the Steering Committee and Alliance partners, assessing governance and fiscal structure options, and helping leadership evaluate potential paths forward for the collaborative.
We conducted partner interviews and facilitated conversations to understand member perspectives, gather insights about the Alliance’s impact, and identify priorities for both how to navigate the transition and what a next chapter that is aligned with the Alliance’s values could be.
Throughout the process, we focused on supporting thoughtful decision-making while honoring the relationships, shared learning, and cross-sector collaboration that had developed over the Alliance’s five years. The work emphasized transparency, reflection, and intentional transition.
